Support coordination is a Capacity Building support that helps you understand and implement your plan — connecting you with providers and building your confidence to manage supports yourself.

What a coordinator does

They help you find and connect with providers, negotiate services, resolve issues and prepare for plan reviews. A good coordinator works themselves out of a job by building your independence.

Levels of support

There are three levels, from Support Connection through to Specialist Support Coordination for participants with complex needs. The level included depends on your situation.

Is it for you?

If your plan is new, complex, or you are feeling lost, support coordination can be invaluable. If you are confident navigating the system, you may not need it — the choice is yours.
